Vector image made by me.

Open your vector image,
Windows, Duplicate and close the original for future use.
Image Resize by 85.
Adjust, Sharpness, Unsharp mask

Effects, Plugins, Xero, Fritillary

Effects, Edges, Enhance
Select your free hand tool and set to Point to Point

Randomly trace around your image 'knocking' out tiles
by pressing delete

Once you have finished
go to your layer palette, right click this layer and select Duplicate.
On the bottom layer:-
Adjust, Blur, Motion Blur

Effects, Texture, Textures

On your top layer:-

Drop Shadow of
Vertical = 5
Horizontal = 5
Opacity = 75
Blur = 5.00

Now add your text, make sure it is set to vector so that we can easily adjust the size and angle.

Select the following areas to turn your name and lengthen and widen your name.

Again, on your layer palette duplicate this layer nad on the underneath layer add the blur settings we used before.
You may want to lower the opacity of this layer.
And thats it done..nice and easy eh!
