OK, start by opening a 500x500 transparent canvas.
Open up your tubes of the Tummy, Holly and Stripes.
Copy and paste your tummy tube onto the blank canvas As A New Layer.
Go to Layers and Add a New Raster Layer.
Using your selection tool, mark a square around the tube

Using your flood fill tool, select patterns on your style box, find your Stripes and fill the square

Selections, Select None
Go to Layers, Arrange, Move Down.
Layers again and Merge Visible.
Selecting your crop tool, crop your image like so,

Effects, Plugins, Plugin Galaxy, Edge Tool

Add your Drop Shadow

Go to Layers and Add A New Raster Layer
Now we need to add your text, using these settings

Effects, Plugins, Eye Candy 4, Glass

Colours, Adjust, Brightness/Contrast, this can be found here

Set your Brightness to 25 and your Contrast to 0
Go to Image, Rotate

Add your Drop Shadow using the same settings as before.
Copy and paste your holly tube three times.
Position the first over the first letter of your name and Add Drop Shadow.
Click on one of the remaining holly's to make it active and go to Layers, Arrange, Move Down

repeat this action till it sits behind your name.
Now for the next action it is best to zoom it to get a closer view.
Place the remaining holly over the one beneath your name and make sure it is lined up.

Using your eraser, remove some of the ribbon to make it look as if it is looped around the letters.
Add your Drop Shadow ..again same settings as before.
Add your watermark if you have one then merge all layers!
All done ...very simple eh!
