OK, first things first.
You need to open a new canvas 500 x 500 transparent.
Open your tube that you are going to use.
Copy and paste onto your canvas.
Go to your layer palette, rename this layer as background.
Duplicate this layer, renaming it tube, then hide this by x out this layer.
Making sure you are active on the background layer
Effects, Eye Candy 3, Glass
using these settings

Effects, Xenofex 1, Shatter
using these settings

Effects, ~FF Gallery B, Bubble
using these settings

Effects, 3D Effects, Drop Shadow
using these settings

Make your tube layer active.
Again use the drop shadow effects shown above.
Position your tube where you feel happiest.
Create a New Raster Layer for your text.
For this I used Nora Casual.
Highlight relevant colours from your image.
I used the following settings for my colours.
Stroke #000F84
Fill #FF9B05

Now go to
Effects, Eye Candy 3, Glass
using the same settings as before
OK.
Now you want to add your Inner Bevel
these are the settings I used

Next you need to rotate your text

Add your drop shadow
Same settings as before.

Go to Selections
Select None
Now add your watermark if you have one
Merge layers
Et voila!!!
