Open blank canvas 300 x 300
Copy and paste your jpg as a New Raster Layer.

Select your pen tool, set as a straight line
draw a diagonal line across your jpg roughly
following the dark line on the picture.

Selections, Select All
Selections, Float
Make sure the jpg layer is active and then hit delete.
Now delete the line layer as we no longer need this.

Add Drop Shadow to the jpg layer

Duplicate this layer.
effects, Plugins, Xero, Fritillary

With your Eraser tool

erase the flower part on this layer.
Effects, Edge Effects, Enhance.

Now paste your ribbon, ribbon holder, cherish word and bow.
Paste the diamond, duplicate 3 times.
Add Drop Shadow - same settings.

Add your name.
Font Scriptina,
Size 36
Foreground - White
Background - White
Bold.

Add Drop Shadow - same settings.

All done.
