This was written in PSP 9 but can be done in other versions.
Open your Jpg.
Image, Resize

Image, Add Border

Image, Resize

Add a New Raster Layer,
Move to the bottom and Flood Fill white.
Make your image the active layer again,
Resize again by 85 %.
Selections, Select All,
Selections, Float
Effects, 3D Effects, Drop Shadow

Selections, Select None.
Open a transparent canvas 600x600.
Copy your thumbnail and paste as a new layer.
Duplicate twice:
(right click the layer name on your layer palette
and choose Duplicate.)
Line your thumbnails up.

Hide the bottom layer of your palette.

Layers, Merge Visible.
Place your image near to the bottom left corner

Duplicate this layer
Hide your duplicate for now and
continue working on your image.
Effects, Plugins, Filter Factory B, Circulator
Duplicate this layer
Image, Mirror.
Move this image to sit
nicely within the circular lines.

Layers, Merge Visible
Resize by 85%
(make sure All Layers is UNCHECKED)
Adjust, Sharpness, Unsharp mask

Un-hide your bottom layer and flood fill black.
Un-hide your top layer of thumbnails.
With your magic wand tool,
click the white surrounding area, then delte.

Resize by 85%
Adjust, Sharpness, Unsharp mask.
Place in the center of your circle.
Open your tube, copy and paste as a new layer.
Image, Resize by 85% and
adjust sharpness same as before.
Add your Drop Shadow

Effects, Plugins, Eye Candy 3, Perspective Shadow

Grab your preset shapes tool and set like so

Draw a circle in your image.
Click your Text tool, and use the nodes
to adjust its shape to outline the inner circle.

Layers New Raster Layer
Align your cursor along the vector line until you
see a A with a curve under it.
Click and add your name.

Add your copyright information and your watermark.
Merge all and resize to taste..
dont forget to adjust your sharpness.
All done!
